De Japanse Onderzoeksraad (JTSB) heeft een rapport gepubliceerd waaruit blijkt dat de piloten van Hokkaido Air vlucht 2891 veel te veel op de auto-pilot vertrouwden wat ertoe leidde dat het vliegtuig tot 30 meter boven de grond kwam. Een crash kon op het laatste moment voorkomen worden.
Go-Around
Op 4 juni 2011 moest een Saab 340B een go-around maken (op 600 voet) van baan 31 op Okushiri airport. Het toestel klom weer tot 750 voet, maar dit werd snel veranderd in een daling. Om 11:38 werden de piloten bewust van de situatie en een bijna-crash werd afgewend. Het toestel kwam tot een hoogte van 90 voet (30 meter) boven de grond. Aan boord waren 13 personen: 2 piloten een flight attendeert en 10 passagiers. Niemand raakte gewond en het vliegtuig bleef onbeschadigd.
Mogelijke oorzaken
De JTSB geeft de volgende mogelijke oorzaken:

It is highly probable that these findings resulted from the fact that the PIC could not perform a fundamental instrument flight, the PIC and the FO used the Autopilot/Flight Director System in an inappropriate manner without confirming the flight instruments and the flight modes, and the FO could not transiently carry out closer monitor of the flight instruments because of the other operations to be done.
Moreover, it is probable that the FO’s operation of engaging an autopilot and changing the vertical mode to make the Aircraft climb by using the Autopilot/Flight Director System eventually became a factor to delay avoiding maneuvers against ground proximity.
It is probable that the Company didn’t create a standard procedure, reflecting the contents of Aircraft Operating Manual, for its crewmembers to confirm and call out the changes mode, without noticing its importance and didn’t carry out adequate training. Furthermore, it is probable that the PIC and the FO excessively relied on the autoflight system.[/su_quote]
Veel groter probleem
Het is een veel groter probleem dat piloten veel op automatische systemen vertrouwen en weinig skills hebben om handmatig te vliegen volgens de Amerikaanse NTSB. Uit een vorig jaar verschenen rapport blijkt dat in een kwart van de ongelukken dit de oorzaak is. Nog veel meer bijna-ongelukken zijn er gemeld, waarbij dit de oorzaak lijkt te zijn.
